By filling out these forms ahead of time you will save significant time on your visit. If you have any questions when filling out the form, please do the best you can, and our staff will assist you with your questions on the day of your appointment.
“As such, we require 48 hours/2 business days notice in the event an appointment must be cancelled. This allows other patients awaiting treatment to be rescheduled into the time slot initially reserved for you. Short notice cancellations (i.e. less than 48 hours notice) and missed appointments are subject to a $100 fee.
